If you have a steely look of determination when this is happening, you are most likely applying too much power on the exit of the corner or even before the apex, where as if you are feeling like you might need to return to the pits for clean underwear then the likelihood is you are going into the corner too fast and upsetting the balance of the car by being on the brakes or lifting off the throttle mid corner or as many people do, leave down changes to late and shock the rear tyres into losing grip.
I've yet to do anything that would soil myself while driving. Not even sure how bad it would need to get to do that.
It's odd, because the speed was right, and I'm sure that I didn't lift off. I'd gone in at the right speed, with a balanced throttle and the car stable, but turned in a little too late. So I put more steering on, to see if I had enough grip to tighten the turn, and that was when the back end slid. I was expecting a little understeer, but not that.
